Farxiga and Ketoacidosis

Farxiga is a type-2 diabetes medication sold by AstraZeneca. It belongs to a class of drugs that inhibit a kidney protein called sodium-glucose cotransporter-2 (SGLT2). It helps prevent hyperglycemia by stimulating the kidneys to remove excess sugar in the urine.

The problem is that the FDA has received 20 reports of ketoacidosis in patients on SGLT2 inhibitors, according to a Safety Communication. The European Medicines Agency (EMA) has reported 101 cases of ketoacidosis from SGLT2 inhibitors.

Unusual Side Effect May Delay  Diagnosis and Treatment

These cases of ketoacidosis were unusual. Normally, the disorder occurs in people with type-1 diabetes who have very high blood-sugar. However, cases reported to the FDA occurred in type-1 diabetes with normal blood-sugar who were on SGLT2 inhibitors. The FDA is concerned that patients on Farxiga with ketoacidosis might not be diagnosed right away, which could increase the risk of life-threatening complications.

Ketoacidosis May Occur After Sudden Illness

According to a warning from the Australian Therapeutic Goods Administration (TGA), sudden illness may occur just before patients on Farxiga develop ketoacidosis:

“In some of the reported cases, just before or at the same time as the DKA occurred, patients experienced acute illness (such as, urinary tract infection, urosepsis, gastroenteritis, influenza, trauma or surgery), reduced caloric or fluid intake, and/or reduced insulin dose.”

What is Diabetic Ketoacidosis?

Diabetic ketoacidosis (DKA) is very serious and hospitalization is usually necessary to bring blood-chemistry back to normal. Most patients receive intravenous (IV) insulin, fluids, and electrolytes.

Symptoms of diabetic ketoacidosis:

  • Difficulty breathing
  • Nausea
  • Vomiting
  • Abdominal pain
  • Confusion
  • Unusual fatigue or sleepiness
