Janumet and Pancreatitis

Acute pancreatitis (sudden inflammation in the pancreas) is one of the most serious complications of Janumet, an anti-diabetes medication that contains sitagliptin and metformin. Janumet pancreatitis complications were first highlighted by the U.S. Food and Drug Administration (FDA) in this Safety Communication from 2009. The agency warned:

“Eighty-eight post-marketing cases of acute pancreatitis, including two cases of hemorrhagic or necrotizing pancreatitis in patients using sitagliptin, were reported to the Agency between October 2006 and February 2009.”

Janumet Pancreatitis Complications

The most serious Janumet pancreatitis complications occur when inflammation is not reduced. Within days or weeks, damaged blood vessels in the pancreas can begin bleeding (hemorrhagic pancreatitis) which can cut off the blood-supply to pancreatic tissues and cause tissue death (necrotizing pancreatitis). These Janumet pancreatitis complications are life-threatening.

Janumet pancreatitis complications may include:

  • Severe inflammation
  • Internal bleeding
  • Obstruction of the bile duct: Symptoms of this may include jaundice (yellowing of the skin and whites of the eyes)
  • Weight loss, dehydration, malnutrition: These complications may be caused by nausea, vomiting, or diarrhea. Malnutrition can also occur if the pancreas stops producing digestive enzymes that are necessary to break down foods.
  • Pancreatic pseudocyst: Occurs when the fluids (blood, pancreatic enzymes, and liquefied tissue) accumulate in a sac in the pancreas.
  • Pancreatic abscess: Pseudocyst that becomes infected with bacteria.
  • Cyst or abscess rupture: This may spill digestive enzymes and bacteria into the abdomen, which is highly likely to cause sepsis and organ damage.
  • Sepsis (septic shock): Occurs when a bacterial infection causes a whole-body immune-system reaction and severe inflammation.
  • Multiple organ system failure: Sepsis can cause small blood clots to form in blood vessels, which can cut off blood-supply to organs and cause severe organ damage and organ failure.
  • Need for surgery: Infected, necrotic pancreatic tissue often must be removed surgically to reduce the risk of sepsis. The surgeon may also need to place a shunt to drain a pancreatic abscess or cyst.
  • Permanent disability
  • Death
