Benicar and Kidney Failure

The Prescribing Information for Benicar includes warnings about post-marketing reports of acute kidney failure and increased blood creatinine levels. High creatinine is usually a sign of kidney disease or impairment. Benicar has also been linked to rhabdomyolysis, a type of severe muscle damage that can cause kidney failure.

Patients who have pre-existing kidney disease should be closely monitored for kidney problems while using Benicar. In high-risk patients, changes in kidney function are to be expected. Therefore, patients may need a blood test to check kidney function at baseline before initiating treatment with Benicar.

In certain patient populations (i.e, those with heart failure), treatment with Benicar may increase the risk of several kidney disorders, including low urine output (oliguria) and higher levels of waste products in blood (progressive azotemia). In rare cases, it could also lead to kidney failure or death.

Sprue-Like Enteropathy and Kidney Failure

Benicar is associated with a severe intestinal disorder known as sprue-like enteropathy, which causes chronic diarrhea and severe weight-loss. Left untreated, patients can become dehydrated and develop kidney problems.

FDA Warning

April 2012 — The U.S. Food and Drug Administration (FDA) has warned that patients with diabetes or kidney problems should not take Benicar and another blood pressure medication containing aliskiren (brand-name Tekturna). This combination can lead to life-threatening kidney impairment, low blood pressure (hypotension), and high potassium blood levels (hyperkalemia). It may also increase the risk of stroke and death.

What is Kidney Failure?

Kidney failure, also known as “renal failure,” occurs when the kidneys are unable to filter waste products from the bloodstream, balance salt and water in the bloodstream, and/or regulate electrolyte (mineral) levels in the blood. In most cases, kidney failure is permanent and patients will need an organ transplant or dialysis (mechanical cleaning of the blood) every few days.
