FDA Safety Warning

December 2015 — After receiving 19 reports of pyelonephritis and urosepsis, the FDA has issued a Safety Warning for patients on diabetes drugs in the SGLT2 inhibitor class.

The agency said all 19 patients were hospitalized and some required dialysis or intensive care. The complications were caused by Urinary Tract Infections (UTIs), a common problem in patients on SGLT2 inhibitors.

SGLT2 inhibitors work by forcing the kidneys to remove more sugar from the body in urine. Patients urinate much more frequently and have more sugary urine, which is why urinary infections and vaginal yeast infections are two of the most common side effects.

What is Pyelonephritis?

Pyelonephritis is an infection in the kidneys. Many types of bacteria and viruses can cause pyelonephritis, but E. coli is the most common cause. Pyelonephritis occurs when bacteria get into the kidneys after traveling up the ureters from the bladder.

Symptoms of Pyelonephritis

  • Fever
  • Nausea, vomiting
  • Pain in the back, side, or groin area
  • Chills
  • Frequent and painful urination

Complications

Sepsis is one of the most serious complications of pyelonephritis. It occurs when bacteria enter the bloodstream. In severe cases, patients go into “septic shock” and experience a rapid drop in blood pressure that causes organ failure.

Kidney failure is another life-threatening complication. It occurs when the infection causes permanent scarring (“fibrosis”), which decreases kidney function. This can lead to high blood pressure, Chronic Kidney Disease (CKD), and kidney failure.

Treatment

Patients with pyelonephritis usually must be hospitalized, where doctors will culture a urine sample to see what bacteria is causing the infection. The patient will receive antibiotics that work best against that bacteria, usually for 6 weeks. They may also need intravenous (IV) antibiotics, fluids, and other medications.
