Overview

Tribenzor is a prescription medication for people with high blood pressure. It contains a combination of two anti-hypertensive drugs olmesartan and amlopidine. It also contains a diuretic, hydrochlorothiazide, to help prevent edema (swelling due to fluid buildup in the legs).

Tribenzor and Sprue-Like Enteropathy

Sprue-like enteropathy has been linked to olmesartan since August 2012, when Mayo Clinic Proceedings published a report from doctors who treated 22 patients with chronic diarrhea and severe weight-loss. The patients tested negative for Celiac disease (also known as “celiac sprue”) but improved when olmesartan was discontinued.

No one knows exactly why Tribenzor is associated with sprue-like enteropathy. Olmesartan is the only drug in its class (angiotensin II receptor blockers or ARBs) that have been linked to enteropathy. It is possible that it is a delayed immune system response or a localized hypersensitivity reaction.

Prescribing Information

The Prescribing Information for Tribenzor contains the following warning about sprue-like enteropathy:

“Severe, chronic diarrhea with substantial weight loss has been reported in patients taking olmesartan months to years after drug initiation. Intestinal biopsies of patients often demonstrated villous atrophy. If a patient develops these symptoms during treatment with olmesartan, exclude other etiologies. Consider discontinuation of Tribenzor in cases where no other etiology is identified.”

FDA Updates Label on Tribenzor

In July 2013, FDA published warnings about sprue-like enteropathy from Tribenzor and other drugs containing olmesartan. The FDA found “clear evidence” linking olmesartan with 23 case reports of enteropathy. In every case, the patients got better after discontinuing olmesartan. In 10 cases, symptoms reappeared when the patients re-started olmesartan.

What is Sprue-Like Enteropathy?

Sprue-like enteropathy is an intestinal disease that causes chronic diarrhea, weight-loss, abdominal pain, nausea, vomiting, bloating, and other debilitating symptoms.

Over time, it can flatten the lining of the intestines (a complication called villous atrophy), which can destroy a person’s health. Intestinal damage may lead to permanent problems with dehydration, malnutrition, and other complications.

Symptoms Resemble Celiac Disease

The onset of symptoms is usually delayed by months or years after a patient starts taking Tribenzor. When symptoms appear, they may resemble Celiac disease (“celiac sprue”), with similar intestinal biopsies. However, unlike Celiac disease, a gluten-free diet does not cure sprue-like enteropathy. Only discontinuing olmesartan will cure the symptoms.
