The Prescribing Information (PDF) for Jardiance recommends that patients should have their kidney function monitored before starting treatment and periodically thereafter. Jardiance is not recommended for people with moderate to severe renal impairment.

The kidneys are two bean-shaped, fist-sized organs located in the middle of the lower back. They are responsible for filtering the blood, removing extra fluids and wastes in urine, and more. In people with diabetes, extra sugar in the blood damages delicate structures inside the kidneys. This damage can develop for 5-10 years before any symptoms appear. However, the damage is often irreversible and can lead to diabetic nephropathy.

Jardiance helps prevent high blood-sugar levels by forcing the kidneys to remove more sugar from the body in urine. Experts are concerned that long-term treatment with Jardiance could lead to kidney failure in several ways.

Dehydration and Kidney Failure

One of the most common side effects of Jardiance is dehydration. It occurs when excessive urination depletes fluids and minerals in the body. Dehydration also decreases blood-volume and blood pressure, which makes it harder for the kidneys to do their job. A sudden drop in blood-pressure can cause acute kidney failure.

Infections and Kidney Failure

Infections are another common side effect of Jardiance. In clinical trials, nearly 10% of patients on Jardiance developed urinary tract infections. In December 2015, the FDA updated warning for Jardiance to include life-threatening urinary infections that spread to the kidneys and cause kidney failure. The FDA received reports of patients who had to be hospitalized and undergo dialysis.

Symptoms of Kidney Problems

The early stages of kidney disease do not usually cause any symptoms. As the damage progresses, toxins can build up in the body and cause symptoms, such as:

  • Feeling tired most of the time
  • Generally feeling sick
  • Headache
  • Nausea and vomiting
  • Poor appetite and weight-loss

How Is Kidney Failure Treated?

People with kidney failure will need to undergo dialysis, which is a machine that filters harmful wastes, salt, and excess fluids from the blood. The only other treatment is a kidney transplant, but most people have to wait years before they are eligible for a donor kidney from a healthy person.
