Invokana and Infections

Invokana (canagliflozing) treats type-2 diabetes by helping the kidneys excrete more glucose (sugar) in the urine, which lowers blood-sugar levels. Invokana is a diuretic that relies heavily on the kidneys. The most common side effects include infections of the bladder and kidneys.

What is the problem?

December 4, 2015 — The label on Invokana has been updated with new warnings about the risk of severe, life-threatening urinary tract infections. Between March 2013 and October 2014, the FDA received 19 reports of patients who were hospitalized with blood infections (urosepsis) and kidney infections (pyelonephritis) that started as urinary tract infections. A few patients developed kidney failure and needed dialysis or intensive care.

Yeast Infections

The most common side effects of Invokana are yeast infections in men and women. In clinical trials, 10% of women and 8% of men on Invokana developed yeast infections. Yeast infections are caused by overgrowth of Candida, a naturally-occurring fungus that is normally balanced by the bacteria lactobacillus. Symptoms may include:

  • Foul-smelling odor
  • White or yellowish discharge
  • Itching, redness, and swelling
  • Pain

Urinary Tract Infections

The second most-common side effect of Invokana is a Urinary Tract Infection (UTI), which occurs when bacteria goes up the urethra and infects the bladder. In clinical trials, 6% of patients on the 100-mg dose developed a UTI. Because urinary tract infections from Invokana can become very serious, seek medical attention right away if you experience the following symptoms:

  • Pain or burning sensation when urinating
  • Frequent and urgent urination
  • Pain or discomfort in the lower abdomen (pelvis)
  • Blood in the urine
  • Fever
  • Back pain
  • Nausea, vomiting

Blood Infections (Urosepsis)

One of the most serious complications of a urinary infection from Invokana occurs when bacteria spreads to the bloodstream and causes a blood infection (urosepsis). Patients can rapidly go into shock, a state of physical shut-down in which blood pressure drops and vital organs do not receive enough oxygen to function. Symptoms may include:

  • Confusion
  • Fever or low body temperature
  • Rapid heartbeat
  • Shaking
  • Skin rash
  • Light-headedness due to low blood pressure

Kidney Infections (Pyelonephritis)

Kidney infections (pyelonephritis) occur when bacteria spreads beyond the bladder to the kidneys. In severe cases, the kidneys are permanently scarred and patients develop chronic kidney disease, high blood pressure, and kidney failure. Symptoms may include:

  • Fever
  • Nausea and vomiting
  • Back, side, or groin pain
  • Chills
  • Urination is frequent and painful
