Safety Warning for Slide Playsets Linked to 1 Child Death

In August 2026, the U.S. Consumer Product Safety Commission (CPSC) issued an urgent warning to stop allowing children to use Model WPX-4 Slide Playsets that were manufactured by Wenzhou Yidian Amusement Equipment Co., Ltd.

The manufacturer refused to agree to an acceptable recall remedy, so the CPSC proactively issued the warning. There was 1 report of a 17-month-old child who became entrapped and died.

This is a “stop use” warning. Because the manufacturer has not agreed to an acceptable recall of the product, there is no refund, repair, or replacement for customers who are affected.

What Is the Problem?

The playsets have multiple gaps that are big enough for a child’s torso to slide through, but narrow enough to trap the child’s head. In other words, a child can slide feet-first or climb through an opening, then get caught by their neck with no way to escape. Entrapment of this kind can happen quickly and silently.

The CPSC describes the hazard as follows:

“The playsets contain multiple openings where a child’s torso can fit through the openings but a child’s head cannot, posing a risk of serious injury and death due to head and neck entrapment hazards.”

How To Identify a WPX-4 Slide Playset

Customers can identify the playset with the following information:

  • Model number WPX-4, found on the product label
  • Manufacturer listed as Wenzhou Yidian Amusement Equipment Co., Ltd.
  • Plastic construction, designed for indoor or outdoor use
  • Includes a tunnel, a slide, and a basketball hoop component
  • Some versions have a vehicle-shaped component on the side
  • Sold in red, navy, coffee, gray, green, and yellow

Because the manufacturer did not issue a formal recall, it is unknown many of these playsets were sold in the U.S. or are still being used in homes, yards, and childcare facilities.

What Should I Do?

Stop using WPX-4 Slide Playsets immediately and dispose of them. Do not sell or give away the product to another family. Consumers who had an incident involving the playset can report it to the CPSC.
