Boston Scientific Recalls Chariot Guiding Sheath

In December 2015, Boston Scientific initiated a recall for the Chariot Guiding Sheath, an access device used to guide the placement of smaller devices into the veins and arteries of a patient’s arms, hands, legs, or feet (peripheral vasculature).

The company received 14 complaints of the shaft of the device breaking. In some cases, pieces broke off inside the patient and emergency procedures were necessary to retrieve the broken pieces.

The recall involves about 7,000 products that were distributed between June and November 2015. Healthcare professionals should follow up with any patients who were treated with the Chariot because shaft fractures may not have been apparent at the time of surgery.

Health Risks & Complications

According to the FDA, small broken pieces can enter a patient’s bloodstream and obstruct blood-flow, which may result in severe organ damage or death. In several reports, the shaft broke when the Chariot was being pulled out of a blood vessel. Some of the most serious health risks linked to the Chariot Guiding Sheath include:

  • Broken shaft
  • Fragment embolization
  • Blood-flow obstruction
  • Emergency surgery
  • Stroke
  • Damage to the kidneys, intestines, or limbs
  • Death
