Lyons ReadyCare and Sysco Imperial Frozen Shakes Recall
The frozen shake recall involves Lyons ReadyCare Nutritional Shakes and Sysco Imperial Frozen Supplemental Shakes that were served in nursing homes and long-term care facilities, according to the FDA.
The nutritional shakes were manufactured by Prairie Farms Dairy Inc. at a facility located in Wayne, Indiana. They were not sold in stores.
12 Deaths Reported in Listeria Outbreak Linked to Frozen CDC Outbreak Investigation
According to the CDC Outbreak Investigation: As of February 21, 2025, 38 people infected with the outbreak strain of Listeria have been reported from 21 states, with illnesses ranging from August 2018 through January 2025.
What Happened?
This outbreak of Listeria is particularly severe, with 37 out of 38 victims hospitalized, and 12 deaths reported from California, Illinois, Indiana, Michigan, Minnesota, North Carolina, New York, Tennessee, Texas, and Washington. Most of the sick people were living in long-term care facilities, or they were hospitalized before they got sick.

How Did Investigators Find The Source of This Outbreak?
This is a long-running outbreak, with illnesses going back 7 years. The CDC said it investigated this outbreak in 2018, 2021, and 2023.
After yet another Listeria outbreak in October 2024 infected 6 people in long-term care facilities, health officials started looking at supplemental shakes as a “product of interest.”
An inspection of the Prairie Farms manufacturing facility in Fort Wayne, Indiana confirmed that environmental samples tested positive for the same strain of Listeria monocytogenes that has been making people sick.
Lyons Magnus LLC Recalls Frozen Supplement Shakes
On February 22, 2025, Lyons Magnus recalled Lyons ReadyCare and Sysco Imperial Frozen Supplemental Shakes. The products were packed in 4-oz. cartons in flavors like Strawberry, Strawberry-Banana, Vanilla, or Chocolate. They were distributed nationwide to long-term care facilities and other institutions. They were not sold in retail stores.
What Are Nutritional Shakes?
Lyons ReadyCare® and Sysco Imperial® are two brands of thickened liquid nutritional shakes for safe swallowing, supplemental nutrition, preventing unintended weight-loss, and digestive health.
These frozen shakes are primarily used in healthcare settings, such as hospitals, nursing homes, or long-term care facilities. They are also used at home by people who have swallowing difficulties.
What Should I Do?
Infections with Listeria can appear within days of eating contaminated food, but it can take months to cause symptoms. This infection can be very serious or even deadly, especially in vulnerable older adults or people with weakened immune systems.
It is important to seek medical attention if you develop symptoms of Listeria, such as:
- Fever
- Flu-like symptoms (such as muscle aches and fatigue)
